Your records carry over. If you cannot get in, request portal access and we will get you reconnected.
Can I do my paperwork before my first visit?+
Yes - complete the new patient information form online and check-in goes faster.
How do I pay my bill?+
Statements and secure payment live in the patient portal. For billing questions, call your office.
Which office should I use?+
Whichever is closest - Delray Beach, Boca Raton, or Hallandale Beach. Your record follows you to all three.
