The content on this Site is provided for general informational purposes only. It is not medical advice, it is not a diagnosis, and it is not a substitute for care from your own physician. Using this Site, reading its content, or submitting a form does not create a physician-patient relationship between you and Personal Physician Care or any of our physicians. A physician-patient relationship begins only when you are accepted as a patient and seen by one of our physicians.

When you submit a form on the Site (a contact form, appointment request, or membership application):
Submitting a form does not guarantee an appointment, a meet & greet, or membership. An appointment request or membership application is not confirmed - and no patient or membership relationship arises - until our team confirms it with you. Our collection and use of your information is described in our Privacy Policy, which is part of these Terms.
Descriptions of Signature Care membership on the Site - including tiers, included services, member pricing on add-on services, and billing options - are summaries for your convenience. Membership is a package of non-covered concierge amenities. It is not insurance, it is not a health plan, and it does not replace insurance. Covered medical services continue to be billed to Medicare or your insurer as usual.
The final terms of any membership are set by the written membership agreement you review and sign at enrollment; if the Site and a signed membership agreement ever differ, the signed agreement controls. Membership fees, billing cycles, cancellation, and refunds are governed solely by your signed membership agreement. Prices, tiers, and included services may change before enrollment; the fee schedule shown at the time you enroll applies.
The Site describes wellness, aesthetic, weight-management, hormone, IV-therapy, and similar services. Descriptions of these services and of any benefits are general information only; individual results vary, and no outcome is guaranteed. Whether a service is appropriate for you is a decision for you and your physician. Prices for these services may change; the price confirmed at booking or consultation applies.
You agree not to misuse the Site: no attempting to breach security, no scraping or bulk data collection, no submitting false or misleading information, and no using the Site for any unlawful purpose.
The Site's content - text, design, graphics, logos, and photographs - belongs to Personal Physician Care or its licensors and is protected by intellectual-property laws. You may view and share links to the Site for personal, non-commercial purposes; any other use requires our written permission.
The Site links to third-party services we do not control, including our patient portal, appointment scheduler, payment and checkout providers, and map services. Your use of those services is governed by their own terms and privacy policies, and we are not responsible for their content or practices.

TO THE FULLEST EXTENT PERMITTED BY LAW, PERSONAL PHYSICIAN CARE AND ITS PHYSICIANS, STAFF, AND AFFILIATES WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS, DATA, OR GOODWILL, ARISING FROM YOUR USE OF (OR INABILITY TO USE) THE SITE. THIS LIMITATION DOES NOT APPLY TO MEDICAL CARE PROVIDED IN OUR OFFICES, WHICH IS GOVERNED BY APPLICABLE MEDICAL-MALPRACTICE LAW, AND IT DOES NOT LIMIT ANY LIABILITY THAT CANNOT BE LIMITED BY LAW.
You agree to indemnify and hold harmless Personal Physician Care from claims, losses, and expenses (including reasonable attorneys' fees) arising out of your misuse of the Site or your violation of these Terms.
These Terms are governed by the laws of the State of Florida, without regard to conflict-of-laws rules. Any dispute arising from the Site or these Terms will be brought exclusively in the state or federal courts located in Palm Beach County, Florida, unless applicable law requires otherwise.
We may update these Terms from time to time; the "Last Updated" date shows the current version. Continued use of the Site after changes are posted means you accept the updated Terms.
