Before you head north
- A pre-departure visit to settle anything open - refills, pending results, follow-ups
- Medication supply review, so you leave with what you need
- A records summary ready to share with your northern doctors
While you're away
- Your record stays active - results and notes from up north get folded in
- With your written OK, we send records and coordinate with your northern physicians
- Telehealth check-ins where state rules allow
- After-hours calls are answered - a PPC physician is always on call, and calls are triaged first
When you're back
- A return visit to pick up where things left off
- Medication reconciliation - anything that changed up north, updated in one record
- Specialist notes and test results from away, reviewed by the team that knows you
