What does primary care at PPC cover?+

Annual exams, chronic condition management - high blood pressure (hypertension), high cholesterol, diabetes, asthma - sick visits, medication management, screenings, vaccinations, and referrals. Labs and EKG are in-house at every office, and imaging and our pharmacist are at Delray - so when your physician orders tests, they usually happen right here, no outside trip.

How quickly can I get an appointment?+

Call your nearest office in the morning for the best selection across all three offices. For urgent concerns, established patients can reach the on-call provider - and Signature Care members get priority appointments as a membership benefit.

Do you take my insurance?+

PPC bills your insurance directly and accepts Medicare and many Medicare Advantage plans. You pay your plan's copay or deductible. Call any office with your plan details and we will confirm before your visit.

What is Chronic Care Management and do I qualify?+

It is a Medicare-supported program for patients with two or more ongoing conditions - like diabetes, high blood pressure, heart disease, COPD, thyroid, or kidney disease. You get a personal care plan, at least 20 minutes of care coordination each month, 24/7 access to a care team member for urgent non-emergency needs, medication review, and proactive follow-up on labs. It is typically covered with little to no out-of-pocket cost, and enrollment is voluntary - you can stop anytime.

Will my records transfer from my old doctor?+

Yes. Bring what you have to your first visit and sign a records release - our team requests the rest. After that, your full history lives in one PPC record that every one of our providers can see.

What if I need a specialist?+

PPC has in-house specialists, including endocrinology and pulmonology, who share your medical record. For outside referrals, your physician coordinates and follows the results back into your chart.

And the care does not stop when you leave. For patients with two or more ongoing conditions, our Medicare-supported Chronic Care Management program adds a personal care plan, monthly check-ins from a dedicated care manager, 24/7 access to the care team, and proactive outreach when lab values drift - typically covered for Medicare beneficiaries with little to no out-of-pocket cost.
