Chronic care management: extra support between visits, explained

Chronic care management (CCM) is a Medicare-covered program for people living with two or more ongoing conditions - things like diabetes, heart disease, COPD, or arthritis. Between your regular visits, a dedicated care team checks in by phone, helps you stay on medications, coordinates specialists, and keeps a written care plan current. The idea is simple: chronic conditions are won or lost in the months between appointments, not in the appointments themselves. Most Medicare patients pay little or nothing for it, and you can start or stop anytime.

Who qualifies and what it includes

Two or more chronic conditions expected to last a year or more. In practice: monthly check-in calls from a care coordinator who knows your case, medication help and refills, specialist and test coordination, a care plan you can actually read, and a direct line when something changes - instead of waiting weeks for the next open slot.

What it is not

It is not a wellness program, not a sales funnel, and not a replacement for visits. It is the connective tissue between them: the refill that happens on time, the blood-pressure trend someone actually watches, the specialist report that actually reaches your primary physician.

Why it matters more than it sounds

Most chronic-disease emergencies are slow failures that nobody watched developing. A care team with scheduled contact catches the two-week weight gain in heart failure, the creeping blood sugar, the medication that was never picked up. Small catches, made early, are the entire point.

Common questions

What does it cost?

Medicare covers CCM; a small coinsurance may apply depending on your plan, and many supplemental plans cover it. The office can tell you before you enroll.

Do I have to sign up?

Yes - it is voluntary, and you can withdraw anytime.

Who will I talk to?

A consistent care coordinator from your physician's office, working from your care plan.
