Transitional care management (TCM) is structured medical follow-up for the 30 days after you leave a hospital or rehab facility. It typically includes a call within two business days of discharge, a visit within one to two weeks, medication reconciliation, and coordination of any home care, equipment, or specialist appointments. It exists because the weeks after discharge are the riskiest stretch of an illness: medications change, instructions get lost, and small problems become readmissions. Medicare covers TCM, and practices that do it well catch trouble while it is still small.
In the hospital your medications may change - some stopped, some started, doses adjusted. Home instructions arrive in a stack of papers on a hard day. Follow-up appointments need booking. Meanwhile you are weak, tired, and supposed to manage all of it. Studies consistently show this window is when recoveries wobble - which is exactly why Medicare pays for structured follow-up.
Someone from your physician's office calls within days to check symptoms and confirm you understand the new medication list. Your discharge summary is already in your physician's hands, not waiting for you to carry it. The follow-up visit reconciles every medication, checks the problem that hospitalized you, and sets the next steps. One office owns the month.
Before leaving the hospital, ask which medications changed and why. Make sure your primary physician's name is on the discharge papers so records flow. Book the follow-up before you get home if you can. And call the office - not the hospital - with new symptoms in that first month unless it is an emergency.
Does Medicare pay for it?
Yes - TCM is a covered Medicare service, and most major plans cover it too.
What if I was in the hospital out of state?
The same logic applies - tell your physician's office as soon as you are discharged so the clock starts.
Is this the same as home health?
No - home health provides skilled care at home; TCM is your physician's office coordinating your recovery. Many patients have both.
